You are required to keep a major credit card on file with us, authorizing charges for account balances older than 30 days and to confirm appointments scheduled in advance. We will always ask your consent and present financial arrangements in writing for any treatment we provide. A fee of at least $50.00 and a minimum of 50% of scTrinity Footerheduling deposit is charged to patients who miss or cancel an appointment without 24 hour notice.
