THE AMERICAN BOARD OF ORAL IMPLANTOLOGY/IMPLANT DENTISTRY, THE INTERNATIONAL CONGRESS OF ORAL IMPLANTOLOGISTS AND THE AMERICAN ACADEMY OF IMPLANT DENTISTRY ARE NOT RECOGNIZED AS BONA FIDE SPECIALTY ACCREDITING ORGANIZATIONS BY THE AMERICAN DENTAL ASSOCIATION OR THE FLORIDA BOARD OF DENTISTRY.
Professional credentials reflect advanced education and evaluation; they do not create a recognized specialty or guarantee a particular result.
Patient payment rights
THE PATIENT AND ANY OTHER PERSON RESPONSIBLE FOR PAYMENT HAS A RIGHT TO REFUSE TO PAY, CANCEL PAYMENT, OR BE REIMBURSED FOR PAYMENT FOR ANY OTHER SERVICE, EXAMINATION, OR TREATMENT THAT IS PERFORMED AS A RESULT OF AND WITHIN 72 HOURS OF RESPONDING TO THE ADVERTISEMENT FOR THE FREE, DISCOUNTED FEE, OR REDUCED FEE SERVICE, EXAMINATION, OR TREATMENT.

Appointments are subject to availability and confirmation. Payment, deposits, cancellations, refunds, insurance, financing and written warranties are described in our office policies and your signed documents.
